Physical Therapy Evaluation:
The goal of the evaluation is establishing an appropriate plan of care to address your primary concerns.
This session includes:
● Listening to your story and concerns including discussion of your medical history.
● What is relieving and what is exacerbating your problem?
● Goals for treatment.

The examination itself may include:
● A comprehensive, hands on musculoskeletal assessment of movement.
● Specific hands on assessment to direct areas of concern.
● General assessment of your gait and functional mobility.

This examination will to help identify:
● What may be causing your problem.
● Other parts of your body and/or movement patterns that may be driving your issues.

It will help provide:
● A physical therapy diagnosis.
● Ideas for continued treatment and for self care.
● Potential referrals to other healthcare providers (if necessary)
● Establishing a treatment plan of care to help you meet your goal.
● Get you back to the activities you enjoy!

Treatment sessions:
Treatment that is individually designed based on the results of the
evaluation and may include manual based techniques/hands on
treatments to treat both chronic and acute musculoskeletal pain,
neck and back pain, orthopedic issues, post surgical care, pelvic
health issues, TMJ dysfunction and tension type headaches.
Specialized training in a variety of treatment techniques
including:
● Manual physical therapy treatments for soft tissues and
joints including soft tissue mobilization/manipulation
and/or myofascial release techinques
● Pelvic health treatment
● Craniosacral therapy
● Visceral Manipulation
● Neuromuscular re-education
● Gentle exercises

All treatment is designed to help with your pain and get you back
to your active self!

All services will be self pay as Active Unwind Physical Therapy will
not be billing insurance companies.

60 min session - $130
90 mins - $ 170

Treatment session:
30 mins: $45
60 mins: $90
